In order to begin the certification process, all patients must register with the Pennsylvania Department of Health. To streamline your certification visit, we recommend you complete this step prior to your physician evaluation. You will need the following to complete the registration successfully:
- access to a PC computer with an internet connection, either a desktop or laptop (not a phone or mobile device).
- a valid Pennsylvania driver’s license or state-issued ID (or change of address card if applicable). Your address should be up-to-date with the PA Department of Transportation.
- a valid email address.
- Click on “Adult Patient Registration”. Enter your information so that it matches the most recent form of your PA ID exactly. When you have successfully created your account, you will be assigned a 6-digit Patient ID number. Retain this number for your records.
