Under the Pennsylvania Medical Marijuana Act, “serious medical conditions” are defined as any one of the following:

  • Amyotrophic lateral sclerosis.
  • Anxiety.
  • Autism.
  • Cancer, including remission therapy.
  • Crohn’s disease.
  • Damage to the nervous tissue of the central nervous system (brain-spinal cord) with objective neurological indication of intractable spasticity, and other associated neuropathies.
  • Dyskinetic and spastic movement disorders.
  • Epilepsy.
  • Glaucoma.
  • HIV / AIDS.
  • Huntington’s disease.
  • Inflammatory bowel disease.
  • Intractable seizures.
  • Multiple sclerosis.
  • Neurodegenerative diseases.
  • Neuropathies.
  • Opioid use disorder for which conventional therapeutic interventions are contraindicated or ineffective, or for which adjunctive therapy is indicated in combination with primary therapeutic interventions.
  • Parkinson’s disease.
  • Post-traumatic stress disorder.
  • Severe chronic or intractable pain of neuropathic origin or severe chronic or intractable pain.
  • Sickle cell anemia.
  • Terminal illness.
  • Tourette syndrome.

In order to begin the certification process,  all patients must register with the Pennsylvania Department of Health.  To streamline your certification visit,  we recommend you complete this step prior to your physician evaluation.   Visit padohmmp.custhelp.com to create a patient profile in the Department of Health’s patient and caregiver registry. You will need the following to complete the registration successfully:

  • access to a PC computer with an internet connection, either a desktop or laptop (not a phone or mobile device).
  • a valid Pennsylvania driver’s license or state-issued ID (or change of address card if applicable). Your address should be up-to-date with the PA Department of Transportation.
  • a valid email address.
  • Click on “Adult Patient Registration”.   Enter your information so that it matches the most recent form of your PA ID exactly.  When you have successfully created your account, you will be assigned a 6-digit Patient ID number. Retain this number for your records.

After you have registered with the Department of Health, you’re ready to proceed with scheduling an appointment for your physician evaluation.   Our visits are performed via a HIPAA compliant, secure, telemedicine portal.

After the doctor certifies you in the state system, look for an email confirmation from the PA Department of Health with your next steps on how to order your medical cannabis patient card. Log back into your PA DOH account with the username and password you used when you created your state account.  Order your patient ID card under the “Make Payment” tab.  The DOH fee for the patient card is $50. Individuals who participate in Medicaid, PACE, CHIP, SNAP and WIC may qualify for a discount.  You should receive your card in approximately 10-14 days after you order it online.

Visit a dispensary to purchase the medication. Once you have received your medical cannabis ID card, you can purchase medical cannabis at any dispensary in Pennsylvania.  We recommend you call ahead prior to your first visit to any dispensary to review their protocols for visits.  See this map for dispensaries in Pennsylvania.
